What is a personal declaration application for?

I've been applying for housing for years. Finally I received a letter telling me I had an appointment to and orientation basically and honestly I can remember what for. I don't know if it's section 8 or if it just low subsidized housing. I guessing by the title of the application packet someone will be able to help me. Thank you in advance.
